How to fill out the Registration Forms

IF USING A COMPUTER (MAC OR PC)

  1. Click on the appropriate registration form (Preschool or Toddler)
  2. DOWNLOAD and SAVE the registration form to your desktop.
  3. Open the file from your desktop using ADOBE.
  4. Fill out the form.
  5. Save the form as your child’s name.
  6. Email to avoncoopnursery@gmail.com

IF USING AN IPHONE

  1. Click on the appropriate registration form (Preschool or Toddler)
  2. Click on “More…” or (“Open in…” if using Firefox) when it pops up.
  3. Copy to Adobe.
  4. Fill out the form.
  5. Share the file (Share Original Document)
  6. Email to avoncoopnursery@gmail.com
